Safety Certificate for Import of Grain Containing Herculex® I

To request a copy of a China Safety Certificate for Herculex I insect protection:

  1. Fill out the form below and press the Submit button.
  2. You will receive a confirmation page which you can use as a fax cover sheet.
  3. The confirmation page includes a link to the printable version (PDF format) of the China Safety Certificate Agreement form.
  4. Print both the confirmation page and the agreement form.
  5. Fill out and sign the agreement form.
  6. Send the executed Agreement via facsimile to the following U.S. fax number: (use country code 001 if calling from China) (515) 535-2102 (Attn: Mike Smith), along with the web generated fax memo cover letter and a copy of your company’s letterhead.
  7. Once your fax is received, your request for a copy of the China Safety Certificate will be processed.
  8. After approval is granted, a numbered copy of the certificate will be faxed to you. You will receive a hard copy of the certificate via US Mail.
